SELECTING A HOSPICE

All hospices are not the same. Remember, you have the right to choose.

It is your choice to not only elect the hospice benefit but also choose your hospice provider. Selecting a hospice provider that best meets your needs can be a big decision. We encourage you to interview multiple hospices to ensure you are comfortable with the organization that will be providing care to you or your loved one. We have attached a questionnaire you may use to compare hospices. This questionnaire includes important questions you should ask when interviewing hospice providers.
