RN Case Manager - South Hills 

Job Snapshot: Registered Nurse Case Manager (RN), is  responsible for the planning, coordinating, providing and documenting care for terminally ill patients from the day of admission to time of discharge. The Registered Nurse (RN) collaborates with patient's physicians and interdisciplinary team to develop and implement a hospice plan of care that meets each patients specific needs.

RN Case Manager - North Hills

Nurse Practitioner-Per Visit

Job Snapshot: Collaborate with team members and with the hospice physicians. Perform comprehensive assessments, Face-to-Face visits, and provide guidance/education when working with patients, families and other members of the hospice team.

Hospice Aide-North Hills

Job Snapshot: Provides personal care to our patients by following their individualized care plans while representing the organization in a professional manner. Care may include, but are not limited to: ADL assistance, mobility assistance, positioning assistance and feeding assistance.
