the importance of being present and offering additional support when patients are entering the final phase of the dying process. Those on our Bridge Program receive additional visits tailored specifically to the needs of the patients and families. Although there is never a set number of visits one can receive, on average a patient will receive an additional two to four visits each day. These visits, provided by nurses, home health aides, social workers, and counselors are utilized for care needs, to ensure comfort, and provide support to the patient and family during this difficult time.
